At Connected Care Services LLC, we proudly provide services licensed through the Department of Behavioral Health and Developmental Services (DBHDS) and funded through Medicaid Waivers. Our mission is to help individuals with developmental disabilities achieve greater independence and quality of life.
Our DBHDS Services Include:
Personalized 1:1 Support:
Assistance with activities of daily living, personal hygiene, and household tasks.
Community Integration:
Hiring compassionate Direct Support Professionals (DSPs) who assist individuals in becoming actively involved in their local communities, building social skills, and fostering inclusion.
Community Connections:
Supporting relationship-building through social activities and meaningful connections.
Community Involvement:
Encouraging participation in events, volunteer activities, and local programs to strengthen independence.
Medical Supports:
Skilled Nursing and Private Duty Nursing available for individuals with authorized needs.
Home Health Services
Comprehensive Home Health and Personal Care for Clients of All Ages
Through our VA Department of Health license, we provide a full range of medical and non-medical home health services, tailored to the unique needs of each individual.
Our Home Health Services Include:
Personal Care Services:
Bathing, grooming, toileting, and personal hygiene assistance
Mobility support and safe transfers
Feeding assistance and oral care
Skin care and monitoring
Medication management reminders
Companion Care Services:
Companionship and social engagement
Meal preparation and planning
Light housekeeping and laundry
Transportation to medical appointments and errands
Respite care for family caregivers
Respite Care Services:
Temporary short-term care for individuals
Relief for primary caregivers with flexible scheduling options
Comprehensive support during respite periods
Skilled Nursing Services:
Our Skilled Nursing Services are provided by experienced Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) who deliver high-quality medical care in the comfort of your home. Services are personalized to meet each client’s individual needs and promote long-term health and wellness.
✔️ Private Duty Nursing – Hourly, one-on-one care provided by RNs and LPNs for individuals with chronic conditions, complex medical needs, or post-hospitalization recovery.
✔️ Wound Care & Post-Surgical Recovery – Skilled assessment, dressing changes, and monitoring to support healing and prevent infection.
✔️ IV Therapy & Medication Administration – Professional management of intravenous therapy, injections, and medication schedules.
✔️ Disease Management Education – Ongoing support and education for managing conditions such as diabetes, heart disease, COPD, and hypertension.
✔️ Pain Management Coordination – Monitoring and collaboration with physicians to ensure comfort and relief.
✔️ Tracheostomy & Catheter Care – Safe and effective care for individuals with advanced medical needs.
